Job Description:
- Identify clients eligible for the programs and services provided; conduct intake interview/screening of clients and requests of assistance
- Undertake the necessary groundwork for proper assessment
- Provide psycho-social intervention, as well as service/treatment planning
- Management of cases including development, implementation, recommendation, and monitoring of case plan for each client
- Facilitates and conducts group works, general assemblies and seminars for the scholars and other beneficiaries of SUFI
- Holds and conducts case conferences on assigned children with the appropriate staff of the foundation.
- Promote mainstreaming and coordinating activities such as community management, mediation and facilitate one-on-one interactions with clients
- Provide client support and monitor service delivery.
- Oversee proper administration of the social services provided to all clients and communities in each program
- Recommend programs that would advance the welfare, well-being and development of the clients and their communities.
- Monitor and assess the performance of each program based on the improvement and development of the communities and the client’s well-being.
- Coordinate and communicate with other agencies and institutions for social service partnerships with the agency

Sorok Uni Foundation Inc. is a non-profit organization working for the empowerment and social inclusion of Persons Affected with Leprosy and Homeless Urban/Rural Poor.
